Current Procedural Terminology (CPT®) is a listing of descriptive terms and identifying
codes for reporting medical services and procedures. The purpose of CPT is to provide a uniform language
that accurately describes medical, surgical, and diagnostic services, and thereby serves as an effective
means for reliable nationwide communication among physicians, and other healthcare providers, patients,
and third parties.

Advertising Acceptance - All products or services are subject to publisher's approval. A prospectus or proof of incorporation for advertisements containing investment opportunities is required prior to closing of the issue.
Publisher's Liability - Advertisers and advertising agency agree to indemnify, defend and save harmless the publisher from any and all liability for content (including text, illunstration, representations, sketches, maps, trademarks, labels or other copyrighted matter) of advertisements printed, or the unauthorized use of any persons's name or photograph arising from the publishers reproduction and publishing of such advertisements pursuant to the advertiser's or agency's order. Publisher is not liable for delays in delivery and/or non-delivery in the event of strikes, accidents, acts of God, or any condition beyond the control of the publisher affecting production or delivery in any manner. Publisher reserves right to hold advertiser and/or agency jointly and severally liable for such monies as are due and payable to the publisher.
Terms - All rates are net rates of any commission.
A 10% late fee may be assessed to invoices unpaid for more than 30 days from billing date, and an additional 1.5% may be assessed after each additional 30 days of delinquency.
Advertisers delinquent beyond 90 days will go into automatic collection processing. No further advertising will be accepted until balance is brought current.
The laws governing the placement of adverising will be enforced under the laws of the State of New Hampshire.
